Purpose

To support individuals and whānau to connect with the health system to meet their health needs.​

This service is to help whānau who are Unenrolled or Unengaged in order to improve their wellbeing;

• Unenrolled

Supporting whānau who don't have a GP or health provider to enroll or select a health provider

• Unengaged

Supporting whānau who have a GP or health provider but don't come in when they need support with their health, to work towards their health goals

• Transport Support with Medical & Specialist Appointments

Whānau who require support to get to medical and specialist appointments only
